Endless forms with personal details, medical history, medical tests, diagnoses and treatments. This limited access to the patient’s records produces three severe implications.

The first is the lack of the essential patient background information, which is critical to the diagnosis and treatment. For example, chronic health conditions may require historical data to assess progression. Old charts scanned as PDF files have very limited use. This data must be displayed as longitudinal data in a graphical form, including data points that can be searched and compared for planning effective treatment. Moreover, since health organizations are still operating multiple disparate IT systems and EHRs, they are frequently forced to match patient records across different locations and systems for a unified view of the patient.

Handwritten forms and data from unstructured reports must be manually entered into a computer by clerical staff, which is time consuming and leads to medical and billing errors, higher operational costs, and lower patient satisfaction. Studies show that medical errors are the No. 3 killer in the U.S.! They claim 400,000 lives each year and cost the nation a colossal $1 trillion annually!

CareXeon’s Patient Onboarding SaaS service is a tremendous step forward, because it enables healthcare providers to access all the relevant information related to their patients as actionable data right in their EHR. Using ONC-HIT healthcare information interoperability standards, we import continuity of care documents (CCD) of patients, from current or former service providers using State and National HIEs. We also collaborate with patients to collect their demographic information or any additional documents that they may have, capturing complete information into PHR repository.

Not all EHRs have the capability to parse and import data from CCDs. This capability is partially muted to avoid importing data that could lead to liability issues. Clinical decisions support systems and quality of care analytics require comprehensive capabilities to import data. We provide virtual clipboard forms and structured health data that are electronically portable to leading PMS, EHR applications. We ensure Data Quality and Data Provenance and provide audit tools.

 Patient Onboarding Process

Patients may Call or use Web Portals or e-mails to contact Doctor office / Hospitals to setup appointment.

Patients contact details are transferred to CareXeon. Our call-center agents answer calls on behalf Practice. Our team will work with patients to set appointment and collect all required details via web and or voice as needed to work with the patient.

Required HIPAA and permission to access health records consent forms are obtained from patient.

Carexeon using patient’s consent to access information, search HIEs and connects to Provider EHR to pull patient’s records. Also patients can upload any documents they may have with them.

CareXeon App indexes unstructured charts, performs ETL, normalizes data and creates structured, actionable data in EHR data store.

Providers shares their  EHR access rights with carexeon. Alternatively, providers could use P2P link to download patient records from  Carexeon’s PHR portal.

We encourage Patients to aggregate all their health records at our portal and  take ownership for their Personal Health Records (PHR) and manage them forever.
